A deck that focuses on +1/+1 counters and proliferate mechanics would benefit greatly from including Bloom Hulk, as it can help to rapidly increase the number of counters on your permanents and players. While Bloom Hulk is a solid card for such a strategy, a potentially better option could be Evolution Sage, which not only proliferates but also rewards you with additional land ramp. Ultimately, whether Bloom Hulk should see play depends on the specific synergy and strategy of the deck, but it can certainly be a strong addition in the right context.

If a permanent has +1/+1 counters and -1/-1 counters on it, they’re removed in pairs as a state-based action so that the permanent has only one of those kinds of counters on it.

You don’t have to choose every permanent or player that has a counter, only the ones you want to add another counter to. Since “any number” includes zero, you don’t have to choose any permanents at all, and you don’t have to choose any players at all.

If a spell or ability puts counters on a player or permanent and then instructs you to proliferate, choosing that player or permanent will give that player or permanent another of those counters.

The War of the Spark set doesn’t include any ways for players to have counters, but some cards in other sets say that a player “gets” a counter of a certain kind. Notably, emblems aren’t counters.
