Thumbscrews could be beneficial in a control deck that focuses on card advantage and disrupting the opponent's hand size, as it provides a consistent source of damage if the opponent has a full hand. However, there are likely better options for dealing damage or controlling the opponent's resources in Magic: the Gathering, such as more versatile removal spells or creatures with repeatable abilities. It checks the number of cards in your hand at the beginning of upkeep, and if you have 4 or fewer cards in hand it will not trigger. It checks again when it resolves and will not deal damage unless you still have 5 or more cards in hand.
